As parents, it is completely natural to feel concerned about your children and young people when you are no longer here.
This Care plan, is designed to capture the knowledge you hold, in a way that can be clearly shared with future caregivers, guardians, or professionals.
It provides insight into your child as a whole person — not just their needs, but their preferences, strengths, and the things that help them feel safe and understood.
Within this plan, you will be guided to share important areas of your child’s life, including:
- Their sensory world — how they experience and respond to their environment, including sensitivities, preferences, and what supports their regulation
- Safety and independence — what they can do independently, where they may need support, and how to keep them safe while promoting autonomy
- Communication and understanding — how they express themselves, process information, and best engage with others
- Emotional regulation and wellbeing — what helps them feel calm, what may overwhelm them, and the strategies that support them through difficult moments
- Daily routines and predictability — the structure, rhythms, and familiar patterns that help them feel secure
- Relationships and connection — the people, interactions, and approaches that are most meaningful and supportive to them
Each section is designed to be completed in your own time, in your own words. There is no expectation for perfection — simply an opportunity to share what matters most.
This plan is not just about future care.
It is about preserving understanding, maintaining dignity, and ensuring that your child continues to be seen, heard, and supported as the individual they are.
